Healthcare Coordination Tool Kit (Through partnership with NMDOH/DDSD)

Healthcare Coordination Tool Kit - The Healthcare Coordination Toolkit is designed to assist individuals, guardians and teams in the proper coordination of healthcare for adults receiving DD Waiver funded supports.

- Health Passport - A Health Passport Packet (part of the Healthcare Coordination Tool Kit), which includes the Health Passport, Master Diagnoses List and Doctor Visit Form, is to accompany an individual to all doctors’ visits, emergency room visits, hospital admissions and any other out of home placement.
